You reflect mental attacks back at your attackers.
When you suffer a mental attack while Mental Mirror is off cooldown, you gain (level based) mental armor (MA).
If the attack then fails to penerate your MA, it's reflected back at your attacker.

Mental Mirror is a mental mutation that allows the player to reflect mental attacks back at their source.
This mutation grants the "Mental Mirror" ability, which is not activated but is rather a passive ability. If a creature makes a mental attack against a target with Mental Mirror, the target's effective MA against that attack is increased by a mutation level-based amount, allowing them to better resist its effects. The damage as well as effects will also be reflected back at the attacker if the attack fails to penetrate the target's MA. Successfully reflecting a damage instance of Sunder Mind will reflect that damage instance back at the attacker and will immediately end an ongoing psychic battle.
If Mental Mirror's effects are applied to an incoming attack, it will be placed on cooldown for 50 rounds. It will not apply its bonus MA or reflect attacks while on cooldown.
